Special Instructions for
Full-Time Clinical Trainee Applicants:
Medical Licensor Information Clinical
trainees who do not have a Texas Medical License are required by
the Texas State Board of Medical Examiners to have an Institutional
Permit to serve in a hospital. It should be kept in mind that an
Insitutional Permit, which is in lieu of a Texas Medical License,
is only valid for practice within our institution. Writing prescriptions
for non-M. D. Anderson patients is a felony.
Institutional permits may be issued to postgraduate
training programs approved by the Accreditation Council for Graduate
Medical Education, American Osteopathic Associate, or the Texas
State Board of Medical Examiners for interns, residents, and post
residency fellows.
Applicants who have graduated from a medical school
approved by the Accreditation Council for Graduate Medical Education
or American Osteopathic Association must submit a completed application
and fee 45 days prior to the beginning date of the program and certification
by the director of medical education of the program that the internship,
residency, or fellowship meets appropriate definition.
Institutional permits are issued for a one year
period and may be renewed up to seven times depending upon the requirements
of the physician's specialty training program.
Physicians holding an institutional permit must
confine their practice of medicine to the teaching program. The
permit may be cancelled if the permit is used to practice medicine
outside the teaching program. Denial of a permanent Texas license
is grounds for revoking or not issuing an institutional permit.

for international medical graduates is provided in the instructions
for international students.
Fellowship Candidate Requirements Before
the interview, a candidate for the fellowship program must have
the following documents on file:
- Completed Biographical Data form (please attach
photograph).
- Three letters of recommendation addressed to:
Director, Educational Programs
Division of Medicine - Box 92
- Copy of doctoral diploma (M. D. or D. O. degree).
- Certified transcript of medical school
grades.
Before his/her period of appointment begins, a fellow
must have the following documents on file:
- Completed Immunization Record (and, if necessary,
supporting documents).
- Chest X-ray report (clear) and certificate of
general good health (within 6 months of start of appointment).
Foreign medical school graduates must present all
of the above-listed documentation AND the following:
- Certified transcript of all college (pre-medical
sciences, mathematics, and English) education.
- Proof of eligibility to practice medicine in
the country of the medical school from which the applicant graduated
(foreign medical license).
- Certification by the medical school dean of the
applicant's graduation from medical school. (This certification
must be on medical school letterhead affixed with the appropriate
medical school seal).
- Copy of valid ECFMG certificate issued in or
after 1986.
If an applicant attended more than one foreign medical
school, he/she must resent all the above-listed documentation AND
the following:
- A statement from all medical schools identifying
all courses of study actually attended at the school's primary
classroom facility.
- Letters of reference from each physician who
supervised training of the applicant that did not actually occur
within the country where the medical school is located.
- An explanation by all medical schools of all
convalidation reflected on the applicant's medical school transcript
to include identification of the other school and the course content.
- Fifth Pathway certificate, if applicable.
- All documents in a foreign language must be
accompanied by a certified translation in English.
Special Instructions for International Medical
Graduates

an institutional permit by the Texas State Board of Medical Examiners
for your parent program, that permit is valid for your elective
rotation at The University of Texas M. D. Anderson Cancer Center.
Institutional permits may be issued to postgraduate
training programs approved by the Accreditation Council for Medical
Education, American Osteopathic Associate, or the Texas State Board
of Medical Examiners for interns, residents, and post residency
fellows.
Applicants who have graduated from a medical school
outside the United States or Canada must submit a completed application
and fee 45 days prior to the beginning date of the program. In addition,
the supporting institution is required to review the following original
documents and certify the review to the TSBME as part of the application
packet:
Medical school diploma and certified translation
Medical school transcript and certified translation
Currently valid ECFMG document
Dean's statement of attendance and graduation from medical school
in good standing
The Board's executive director may, on a case by
case basis, allow substitute documents where exhaustive efforts
have been made to secure the required documents.
