Diffuse necrotizing leukoencephalopathy

Case 23  

A 22 year old male with ALL

Shortly after receiving intrathecal MTX and cranial irradiation, this patient developed personality changes and drowsiness. MRI (T2) demonstrates obvious periventricular hyperintensity consistent with necrotizing leukoencephalopathy. There was no enhancement. It may be that the term"necrotizing"should be reserved for the most severe cases or those that do not resolve. This patient was lost to follow up.
