Case 21 and 22
Case 21

In case 21, a 13 year old boy with ALL developed aphasia and discoordination coinciding with a cutaneous herpetiform eruption. MRI (21) demonstrated multiple white matter lesions which were hyperintense on T2. These proved to be Varicella-Zoster leukoencephalitis (case courtesy of D. Enzmann, Stanford, CA).

 

Case 22

In case 22, a patient with CLL and right face Zoster infection experienced V2 distribution pain and developed a right-sided third nerve palsy. MRI (22) demonstrates intracranial spread of Zoster along V2 into the cavernous sinus and Meckel's cave (arrows). This patient responded to acyclovir but shortly thereafter died of a massive intracerebral bleed.
