Case 16 - Adenoid cystic carcinoma of left lateral wall of nasopharynx with extension to maxillary division of trigeminal nerve and gasserian ganglion

Biopsy of the tumor mass of the nasopharynx revealed adenoid cystic carcinoma. Because of the extensiveness of the lesion patient was treated first with chemotherapy and then followed with radiation therapy.

Axial CT scan with IV contrast medium at the level of the upper nasopharynx: soft tissue tumor mass of the left lateral wall of the nasopharynx involving the torus tubarius and fossa Rosenmuller (arrows). Coronal CT scan with IV contrast through the midnasopharynx: soft tissue tumor mass arising from the left lateral wall and left side of the roof of the nasopharynx (1) with destruction of the medial aspect of the greater wing of the sphenoid bone and extension to the left cavernous sinus (2). The left foramen rotundum is involved.

Axial T2-weighted MR image (TR 2000/TE 80) at the level of the mid sphenoid sinuses: tumor mass with intermediate signal intensity in the left cavernous sinus (1), gasserian ganglion (2) and trigeminal nerve (3). There is medial and slightly posterior displacement of the left internal cartoid (4). There is an indentation on the anterior and left lateral aspect of the pons by the tumor mass (5).
