METASTASIS - The Trigeminal Nerve
Now viewing: Case 5
Case 5 - Neurogenic sarcoma of left mandible with metastasis to
mandibular division and gasserian ganglion
|
Operative findings: base of skull sarcoma
involving floor of middle fossa, cavernous sinus, petrous ridge and Meckel's cave with
extension into posterior fossa and compression of brain stem. A subtemporal craniotomy and
transtentorial partial resection of intracranial sarcoma were performed. Histopathology
revealed spindle cell sarcoma suggestive of neurofilerosarcoma. |

Axial CT scan with IV contrast medium at the level of the
mandible: destructive lesion of the middle part of the left mandibular canal with
extension to the regional soft tissue (arrows). |
|
Coronal CT scan with IV contrast medium at the level of the
ascending ramus of the left mandible: most part of the ascending ramus is destroyed
including madibular canal and foramen with regional soft tissue involvement (1). There is
enlargement of the left madibular division of the trigeminal nerve at the base of the
skull (2), enlargement and destruction of the left foramen ovale (3) and extension of the
lesion to the V2 and V3 in the left cavernous sinus and the gasserian ganglion. |

Axial CT scan with IV contrast
medium at the level of the base of the skull: enlargement of the left gasserian ganglion
and the trigeminal nerve due to perineural extension of the lesion is well shown (arrows). |
