POST SURGICAL FLUID COLLECTIONS

POST OPERATIVE SEROMA

57-year-old female with synovial sarcoma of right thigh.

(04/22/91) Axial T1W MR image reveals intermediate signal intensity mass in right thigh (arrow on JPEG image). Patient underwent wide local excision

(08/01/91) Axial T1W MR image about 3 months post surgery reveals seroma which is of lower signal intensity than adjacent muscle (arrow on JPEG image). When seromas exhibit intermediate signal intensity secondary to proteinaceous material, I.V. contrast may be administered to differentiate the fluid collection from solid masses.

(08/01/91) Axial T2W MR image reveals well-defined high signal intensity fluid.

(12/30/91) Axial T2W image obtained approximately 5 months after the prior MR study reveals increase in size of the fluid collection. Seromas may increase in size and this is not a sign of recurrent neoplasm.

(12/30/91) Axial post contrast T1W MR image confirms low signal intensity fluid collection with peripheral enhancing "inflammatory" rim.

(01/08/92) Sonogram obtained 9 days after the MR examination reveals hypoechoic fluid collection. Ultrasound guided fine needle aspiration revealed serous fluid with no evidence of malignancy on cytological examination.

(08/12/92) Axial T2W MR study obtained approximately 8 months following the previous MR study reveals decrease in the size of the seroma. Seromas may be retained at the postoperative site for extended periods.
