MIMICS OF SOFT-TISSUE SARCOMA

Infection and Granulomas

Patient 17

42-year-old female with surgically proven foreign body granuloma in left arm (similar surgically proven mass was noted in opposite arm).

T1W MR image reveals lobulated inhomogenous mass with areas of slightly high and low signal intensity. Patient had a history of being struck by a baseball bat as a child.
2.
T2W MR image reveals low signal intensity mass with nodular areas of high signal intensity. The appearance is nonspecific. We considered desmoid fibromatoses in the differential diagnosis. Foreign body granuloma was not considered prospectively. Histologically fibrotic and necrotic areas were noted within mass.

Gross photograph of resected specimen.

Polarized light photomicrograph from lesion reveals birefringent foreign material within macrophages. (arrow on JPEG image). (Original magnification, X 200 H-E stain). The type of foreign material could not be determined.
