Registration and Fee

Design the course to your specific needs. Enroll for the entire course, including all of the features listed in the brochure, for only $295.00! Or register for one or more of the single subject sessions for only $100.00 per session. Single subject registration includes: review of specific discipline, study guide, practice problems and question and answer sessions for specific discipline.
Your registration must be postmarked by June 4, 1999. The fee payment must be submitted with your registration form. You will receive preprogram materials which include a detailed program schedule, practice case studies, and a map of the UTMD Anderson Cancer Center campus. Registrations postmarked after the June 4, deadline will be accepted only if accompanied by the $25.00 late fee.

Cancellations


To cancel your registration, call The Program in Medical Technology Office at 713-745-1688 by June 4, 1999. Cancellations made by this date will receive a 50% refund. No refunds will be issued for a cancellation made after June 4, 1999 Stopping payment of a check will not constitute formal cancellation. Allow three to four weeks, from the date of cancellation to receive a refund. The program reserves the right to cancel (with full refund) any program due to insufficient enrollment or other administrative reason.
