Case 17 - Squamous cell carcinoma of posterolateral wall of nasopharynx with extension to mandibular division of trigeminal and greater superficial petrosal nerves

Biopsy of the tumor mass revealed squamous cell carcinoma. Patient was treated with chemotherapy and radiation therapy.

Axial CT scan with IV contrast medium through the upper nasopharynx: soft tissue tumor mass involving the right posterolateral wall of the nasopharynx with obliteration of the torus tubarius and fossa of Rosenmuller (1) and extension to the right parapharyngeal space (2). Axial CT scan with IV contrast medium at the level of the base of the skull (bone window): destructive lesion involving the right side of the clivus, right foramen lacerum, petrous apex, and the medial aspect of the right foramen oval. (1). There is widening of the right pterygopalatine fossa (2), right pterygoid canal (3), and increased density of the right pterygoid plate from tumor extension (4).

Coronal CT at the level of the foramen ovale - a large soft tissue mass involving the right side of the nasopharynx with extension to the right base of the skull (1). There is a tumor mass extending upward with destruction of the foramen ovale and possible involvement of V3 (2).

Coronal T1-weighted MR image (TR 800/TE 25) at the level of the foramen ovale: a large tissue mass with low to intermediate signal intensity involving the right side of the roof and right lateral wall of the nasopharynx (1) with extension to the right parapharyngeal space displacing the medial pterygoid muscle slightly laterally (2) . There is extension of the lesion superolaterally to the region of the foramen ovale) with involvement of V3 (3). There is also superior extension of the lesion with destruction of the right basisphennoid and right pterygoid canal and perineural extension of the greater superficial petrosal nerve (4).
